I hereby certify that I am above the age of 18 (eighteen) years and that I am in good health to participate in the cricket coaching/training programme (“Programme”). I fully understand and acknowledge that my participation in the Programme carries foreseeable and unforeseeable risks, including, without limitation, the risk of injury, serious bodily harm and death. Knowing these risks, I agree to assume all risks arising out of my participation in the Programme. To the maximum extent permitted by law, I hereby release and hold harmless SIX and its coaches, personnel, staff, representatives, employees, agents and any other persons or entities associated directly or indirectly with SIX from and against any present or future claims, demands, liabilities, costs and expenses to my property and for any and all illness or injury to my person, including my death, or for any other damage, which I may suffer, or for which I may be liable to any other person, in relation to my participation in the Programme.

Further, I acknowledge that SIX may use my name, image, likeness and any photographic or audio-visual footage recorded from the Programme (which may incorporate or involve me and my personal attributes) in the course of promoting SIX and/or the Programme and communicating about SIX and/or the Programme. I hereby consent to and permit such use of my personal attributes (including my name and image) by SIX.

I hereby agree to comply with the Trainee Terms and Conditions for the cricket programme as laid down by SIX. I understand the fees once paid are neither transferrable nor refundable under any circumstance.

If you are an adult registering on behalf of your ward, please read and agree to the following disclaimer:

I hereby certify that my ward is in good health, and I approve of my ward participating in the cricket coaching/training programme (“Programme”). I, on behalf of my ward, fully understand and acknowledge that my ward’s participation in the Programme carries foreseeable and unforeseeable risks, including, without limitation, the risk of injury, serious bodily harm and death. Knowing these risks, I, on behalf of my ward, agree to assume all risks arising out of my ward’s participation in the Programme. To the maximum extent permitted by law, I hereby release and hold harmless SIX and its coaches, personnel, staff, representatives, employees, agents and any other persons or entities associated directly or indirectly with SIX from and against any present or future claims, demands, liabilities, costs and expenses to my ward’s property and for any and all illness or injury to my ward’s person, including my ward’s death, or for any other damage, which my ward may suffer, or for which my ward may be liable to any other person, in relation to my ward’s participation in the Programme.

Further, I acknowledge that SIX may use my ward’s name, image, likeness and any photographic or audio-visual footage recorded from the Programme (which may incorporate or involve my ward and my ward’s personal attributes) in the course of promoting SIX and/or the Programme and communicating about SIX and/or the Programme.
